I had a medical supplier decide to pick and choose what was sent in with regard to a claim and now another appeal has to be filed as what was givin was not sent in as if the patient knows nothing. So if anyone looks on the system and sees a denial code on there, please be aware of how it occurred and that its not a denial for medial necessity to mdiciare but a request for the information the claims processor needed.. Who is training these billing personnel? HR 1063 has fines which can be assed for billing medciare when ther are other primary payers to be be billed first.
In the Dallas Region much of this has been coming from the Medicare regional office and govt officials have been gravely misleading the billing services and its part of how a trillion dollars ( estimated amount by 500 hundred named witneesses who helped document this over the span of two years inside Vangent -General Dynamics, one of the Partners of medciare)Then did not forward the best evidence into OIG Director Leavenson in violaion of their federal contract and so the horrors and thefts and'real' death panels are on going.
Six months is up soon another round of filings will need to be made via certified mail and so Christmas presents will in part be certiified mail reciepts, again.
OIG FOR HHS lambasted Office of Civil Rigghts for wrong doings. Part of that is not identifying Civil rights complaints and permitting them to be read and answered in any way. Then no regular appeal rights on them which is supposed to occur. Since Govt officials and partners of the govt ( govt contractors) are still not under the HIPPAA Law all we have for "redress of grievances" is civol rights if one is in a
protected' class As I; an old, disabled woman who considers what is occuring by HHS and partners also as retaliation for pending Unsettled HHS civil rights complaints as an employee of the Dept. I fall into several pf the protected classes.
The issues that have been raised since 1/08 cover several broader issues and most seem to be part of a 'class' of patients who have been wrongly affected and reports are too many have died as the issues were never allowed to be addressed and corrective behaviors addressed as the suprmem directive fo the Civil rights laws is corrections and gaiaing fo rights although monetary dmamges can be awarded and s asked for, too. which can never rplace the finacial loss. et along oss of well being and life and physical health lost in all of this.

This new filing amends, revises and protects the filing deadlines for all previously named offenders and amends the witness lists.Its filed by all three of us. Daily notices of offenses have been snet to Debbie Campos althogh she has never been allowed to review and answer the Civil rights complaints filed since 1/08 over the years is what she told me.Se was only givin the HIPPAA complaints which do not apply to almost all of the named offenders. Reg Manager Ralph Rouse has pernsally not allowed the civil rights complaints to be assigned to any of his employees. accoridng to numerous of those who wer given Hippaa compaoints filed only and nothing could be done as it was a civil rights matter. Others At Office of Civil rights each day, seems to be another round of 1 or more instances
Plus the medical profession has spent billions trying to purchase and comply with regulaltions that are discrminatory and make no medical or common sense and in many ways are physcially harmful to all and interfers with all of us getitn gthe best health care scince has available for us all to have the best health and longer lives. I choose life and even though recent events over the last 180 days has weakened internal organs, I still shoudl be able to live a good number of more years. The Let her die order is a violation of my civil rights. And Freday last I asked Ptrick McFarland to see what he coudl do to get that horrific standing order done away with. And the on going pattern of behavior by some eentities named shows well its in effect. I thsnked him for 'blowing the whistle' last &/13 in a Congressional Hearing and hoepfully Congress nd the Presdent will pay heed to him as our lives andstoppiong thefts of public monies are at stake. We all pay taxes either directly or indirectly as consumers in this nation.I pay taxes when I pay the bill for this internet service which is on my phone bill,etc.
Big money seems to have had undue influence in the whole process and we as modest income patients have no where with all to fight against them who are putting our very lives at risk.
Before all of the USA come under this cabal, we need some of these issues addresed and common sense and human and civil rights allowed to prevail and be the motivator which too often is not. If congress will not addressthis, then the civil rights complaint is one way for the issue to be brought forward.
One of an over all bigger problem is the FDA and HHA, etc that continually discriminates agaisnt those who have chemical/ or inflammation from irritants sensitiviities or NON- allergic reaticns that can be deadly and badly harm the physiacl well being of the patients. Some studies put the number at 10 million in the USA, but common sense says its more than that. Its just that the harm done has not been medically diagnosed yet. In the cuurent discriminatory arena, few more may be diagnosed unitl their physical well- being is so bad the truth of cause of problems will need to be addressed and in the current system, my never be addressed as they get worse and die.
Reports are that the USA spends the most on health care, yet has third world country treatments and it goes back to the MONSTER IN THE ROOM ; those chemicals and toxins which our officials say do not harm people in the USA but do in other nations of the world when due care and prudence is not used as to how they are used and refusing to recognize that sometimes the wrong mixes occur and terrible injuries occur. Nd if the enet is crimnal negligence or intentional harm, to often the whole ends up in a horrific cover -up as the liablity insures try to get out of paying any beenfits at all for not using due diligence and prenventing the injuries as turning off the gas alarms in our buidling at 2 Journal Square Jersey City, NJ so the fire dept didn't write another citiation for unsafe conditions rather than fis the problems in a new building which partially got done after workers got badly injured and some have and most will die of their injuries at some point in time. So all the ilelgal proertyary secret deals get made among the insurance industry to ox bstrut them ever having to pay any kind of real damages.and the govt officials seems to be on ther side instead of on the side fo the workers and the law and our rights as cimrnal acts get worse and worse and the cover ups continue along with obstructions and all one can do is ge on the internet and share what is going on with any one that will pay attention that what one perosn is going through is very common place and shoudl not be occurreing. NO FAULT HEALTH instucane might work if the govt and its partners were o the side of jsutice and life. If this had occurred the Jersey City Firs dept woudl have follwoed up and seen problmes were corrected when we in the building had no idea there had been a citation or that toxic gas was being used at all.back in 1988-89 when these modern buildings built for large main frame computers and not for humans were beginning to dot the landscapes of the nation and world and those incharge of our safety had to make sure our lives were protected from harm and not let other toxins and chemicals to get in the mix and cause death and hrrendous injury. In fact, the building we were in, owned by Hartz Mountain Industries could not have egally passed city, state or federal inspection for us to be in it. Laws and regualtions are in place for the purpose of protecting life, not taking it away.... .? Deaths are usually called felony murders in most legal systems in cases like this and no statute of limitations on prosecutions occurs when felony actions put us in this buidling in the first place and it only took six months for major disaster to occur. What is the USA laws?

Chemicals! These products have allowed us all to have a higher standard of living, but when the wrong mix occurs physical harm can occur and usually does in ways we may still not be aware of. When employers and owners of buildings are negligent in even using common sense with regard for human life, then our current legal system as practiced now requires the individual to take on those of great power and wealth and its an impossiblility. Which is what Federal workers compensation requires the modest income worker to do. Most feds are of modest income and usually in poverty once they are injured and not able to work and earn their middle income salary and if they were not under their own health care plans, would be qualifying for medicaid, too along with others now filing.

MEDICAID a sub standard , unequal medical care with such stringent care that most medical providers will not accept the patients and look for one of the next laws passed be that doctors will be required to accept a certain percentage of them as patients. In some states a major percentage of patients may have it for their health plan next year.As it beomes the ehealth plan of the middle class and not just for the poor.
Many doctors won't even take but a few medicare patients due to reductions in payments wich the on going fraud inside the govt's own partners has been allowed as the monies to disappear and no criminal oversight or investigations can occur as Congress has appropriated no money for the partners ( or seen money is used) for the partners or govt contractors govt to be investigated nor prosecuted. except in rare cases of notoriety that gets mass media attentions such as NSA govt contractors spying and hiring those with falsified background checks to do the spying on all of us,etc.
HR 1063 signed into law 1/11/13 puts the responsiblity for this enforcment in medicare which sets the over all tone for ethical and legal us of all of health care dollars) back onto the govt, but its not yet been implemented and few seem to understand what the law really is about as no enforcement of the conditional payment law has been done since 1994 when the Clinton Adminsitration decided that the Healh insurance industry could 'police ' itself and the collapse of our health care sytem really started in earnest as our tax monies disappeared along with our human rights of life which mean health care in this modern era.Congress was too busy reading a porgraphic Star report to stop what was happening under their watch, lot worse than Bill and Monaca's 'antics' in the oval office. I've often wondered if that was a 'smoke screen' to keep the good investigative work on going in many committes to get stopped. and did. Cong Dan Burton and others don't seem to hae been allowed to chair committess since then.And corruption continued , got worse and off to war we went again to find 'one bad guy' sitting in his house in Pakistan. while the world killed each other.and still is.Our Health care dollars at ork in destructive uses and more horrific injuries to be treated.

So addded to all the paper work, is another medicare appeal and send in the info they should have had in the first place.. The provider had decided to ' just absorb the loss and hadn't even sent it on to the primary health plans as the info was given them incorrectly from another medical provider. They do not pay for a 'clearing house' and I told the billing person, you will just get fraudulent and incorrect info any way as the officials posting is getting altered by Emblem Health Internationsl whose subsidiaries are Medicare coordination of benefts who has a contract to get the official posting on me and it can't be enforced according to US Dept of Laor contract pcomliance decision. at all as its another unacocountable Partner fo the govt? Its HHS has to go after them and fine major decisosn, I o Gpt spelled out clearly the need for HR 1063 and now implement it. e them under HR 1063. US dept of Albor can;t go after ther own contractor ACS which is my first promary payer as they don;t have a law for them nor money? So HHS has to fine US DEPT OF LABOR? for their out of control contractor, ACS-Xerox? whch won;t sen Medciare back the money spent on my health care that is already approved at Federal workers comp?And CEO Lynn Blodgett has the refund letters thrown away and told MCOB not to send? any more as ACS-Xerox is also a medicare partner. out of control which has control in one way or another of the total of all of this.Even atorneys for the workers don't seem to be aware of the chaotic, unsconstitutional and illegal actions on going on their own clients?One asked me some time in the last year, who is Lynn Blodgett? I said he is the one giving the order that won't let mine and others' cases to get legal and constitutional reviews and judges remand orders thrown away and bblatantly defied. rather than orders carried out as agency officals can't get access to the files or the orders . as he orders them kept away from them. and no one does anything to enforce these govt contracts that can't be legally valid i such f illegal actions are allowed. ACS -XEROX directly or through interlocking cabal of companies. and subsidiaries. from US Dept of labor on myself and dependents on my Federal Employer group health plan which is being altered in deifince of Meedciare;s appelalte jduge that upheld the positng form US dept of Labor is the accurate one. So soon after posting its ilelgally altered so medicare can be made primary ( felony theft) RICCO case accroding to US attorney;s office which can do nothing as no federal law enforcment is allowed to do anything to refer the case, either. and the 'deadly' computer nightmare records is deadly as diagnosis codes have been changed to symptoms of the accepted medical conditins for Workers comp to circumvnet so extent of injuries stays hidden from view including my being on oxygen for life. So this keeps cliams from being sent in as required but then Affiliated computuer Services-Xerox has orders from CEO Lynn Blodgett to not post post my file so agency officials can't see it to act on it as 15 judges and 2 hearing officers have said to do this or that over the years and ususally the file disappaered again after processing and payments were getting paid by who? Now its the govt partner in charge of life and livelihood and officils can do nothing even when their own apellate judges orders it done as they are not allowed to have the file to work it. . He just tells them what he wants them to know and they have to follow His verbal orders and not the apepallate judges who ordered my file posted and processed, paid, etc back in 2009 and never occurred. That includes the request for permanent disability pending since 1996 with all the medicals, etc to justify it,etc and known of and referred to by a claims examiner who retired in 2009 and sent me a letter recapping some of this that protects my rights the best she could but could not rule on the matter without the file and no way to reocnstruct the file as Lynn Blodgett has to post it for any one to work;as she ws well aware of what was occurring but had no power or authority to right grievous violatitons of civil wrongs on going.Now the last respnce of mine to file areconsidieration over two years ago has never been given to the agency and no response, so they keep giving out false info that everything is over and done with as " o'ut of sight is out of mind' means govt contractor doesn't let us see it--it doesn't exist so the federal judges in all the agencies might as well go home if their orders are to be ignored./thrown away? where the govt ocntractor wears one hat at US dept of Labor and another at SSA, my employer: ( they don't even know it due to hacking into the system documented and no concern even when the head - Deputy Comm begs for it to be addressed-- and another 'hat' over at Medicare, etc. and its on- going conflicts of interest and destruction of human rights all over not only for me and others. d
There is the decade+ of oxygen claims not input , hidden from view his own manager said would be paid automatically by the system if input and then the 'gig would be up' and all my bills would be paid and isn't that what is legally to occur when one has permanent medical on an established case almost 25 years old and most bills are for the same treatments or similar medical care as already approved and paid for in the past.
Many states are now or have been adopting this form of workers comp in your area, so watch out for these aubses to spreead to all workers very soon.Ok Law is in the appelas court as unconstituional. Its amatter of acountability that those in charge obey the rules and apply the lws justly and failry and the opposite is allowed to occur and having legal help does no good. My case is appreoved and established and what more can an attorney do? Like the worker, they ca';t arrest any one, cancel a govt contract etc! Even a judge is ignored so what can the rest of us do?
Then there are the rest of the medical issues that can end life - the secondary and tertiary issues that have arisen from the initial injury and is medically known to occur that have never been allowed to be seen by any one at the US Dept of labor as CEO Lynn Blodgett's manager said only one bady part is allowed to be injuredaccording to past OPM Director to " keep down costs'. So one can breathe but when the heart or kidneys stop or sight ends? what then?.
She said inputting my oxygen claims that gave me thepeprmanent medical benefits can end if no claims are ever allowed to be seen again. DEATH BY COMPUTER? The orginal death panel And no order from any judge be allowed to be seen , either that would stop this. No civil rights case is reviewed to end it and right of life ends. In my case the last order gave instruction to quesiton the renewla of the 5 years contract with ACS-Xerox. Orders not seen and contract renewed so until 2015, Lynn Blodgett can say if I live or die? I pray for him, that he looks into this and does what is legal and right and stop this death panel. People die when leife sustaining medical care is withheld.
Even some attorneys that are helping Fed workers, do not understeand that evn judges orders can't be seen by the cliaims exminer,( Even taking it in an showing it in perosn is NOT OFFCIACLLY SEEN TO BE ACTED ON) real govt officials can' do anything unless the govt contractor allows them to see it. In my case: I am under a "let her die" order and even judges orders can not be officially seen to act on and follow and aren't and intially the ACS call center persons were reading the million dollars of set side money? on the screen telling providers and suppliers I had the money to pay for my own care and did not and none of us serously injured do. Isn;t this called organized cirme attempted murder and some get arrested for such behavior? But not in the USA?
In tryig to find out where this million dolars was that had to be found to get medcial care paid for; That led to Patrick McFaland's OIG OPM office investigator who already was well awre of an 'embezzlement ring' tthat needed to be investigated and stopped and people arrested, etc. and to this date has not been and this is the money for life sustaining medical care for the most seriously injured Workers.Have I already been placed on permanent disability in the computer system at Federal workers comp without any schedual award for loss of body parts use or money paid for wage loss montly or a lump sum and if a lump sum is shown where did it gio and who stole it? IS THERE MILLIONS STOLEN IN MY NAME BY SOMEONE while I live in poverty and struggle to pay for oxygen and health care for life.
The official system does not show it as it posts employer group health care plan as other primary health payer and should not if I had retired which can't occur until placed on permenent disability by Federal workers comp. BUT THERE IS ANOTHER offline system in the Dallas regional office that is not tied to the official one and it may show this and why wrong info given out and OIG for Dept of Labor needs to take a look at it. A hearing officer ordered the Dallas office in 1999 that the system was not legal as it paid items in a lump sum and I had the right to file an appeal on any one itme or part of and one could not tell what was for what and if all were paid in full as requested. This system was still in place and being used in 2009 even though ACS- Xerox is to pay bills as some were paid then. And it was obviously offline. These offline systems with little securuty controls is part of the problem? with the 'clou'd needed to set up the new health care systems for next year and programers of the software are running into all these offline systems that should not legally exist. All kinds of valid checks and embezzlement can occur when this sytem is allowed to exist. I was glad for what did get paid, but it didn't go through ACS-Xerox so the call center ACS employees are not aware of what the Regional offcie even did. The claims exominer retired soon after the check was sent. Better to do away with the contractor and modernize the system and pay using corect procedures, then full accuntability can occur. Someone tired to help? but in the long run it didn;t end the on going problme of unethical and criminal behavior.More reason to ask me for the facts of the case as the judges ordered in 2009 as no one seems to know the whole at all even at Federal workers comp.
However. not everything is itemized on that system as I found out when some bills got paid in a lump sum and no tiemizition by date of service and amount. Is the million on that system , too? Or other huge amounts?Beond some medical bills.? Often this money has gone through the agenices and federal reerve without any formal letters going out to notify attorneys and workers( half the time attorneys don't even get a letter as records in such a mess and incomplete or non existent.)
In my case when home oxygen got coded in for life 18 years ago the money went through and disappeared? on its way to OPM or inside OPM? so that we could easily go for medical care and show our employer group health card or choose one to be billed and they would pay the bills at 100% for accepted charges. This is in information given out to federal workers and was commonly known and told, but no one has seen the money. Yet it caused oobstruction of life giving care in too many instances and some have died a as I've been told from this 'theft' This is under a 1988 law so that permanent medical could be awarded on an established case before any decision woud be made on work or permanent disability. So was the horrbile disaster with subsequent loss of life and years of treatment of terrible injuries in our office a $90 million embezzlement and the first incident to use for the thieves to steal in 1/89? !988 the year ACS was incorporated and started getting those govt contracts which is now the ssme finacially interlocked cabal that has control of the flow of information and thus control of govts around the world? What I have reported here is publically available and the conern is that the new Asian trade agreemnt may see the coup d'etat of China, etc. by them.
I don't fear a wold govt with a constitution and individual rights and rights of sovereogn nation states much as our constitution has. But we have not gotten that. We have a take over by a few powerful, made powerful by their wealth and not the vote of each of us as to whom should be our leaders.

When Mitt Romeny ran for President , the bio of Ann Romney was given out, too. And I recognized much of the story from one of my Dad's sisters as referring to a relative. I am checking into this further, but in 2012 was Pres Obama being opposed by an in-law? Most of my time spent here since 7/26/12 was in sharing my eye witness account of events of the President's birth in Topeka , Ks and the non issue of his eligibility to be president due to be ing born in USA. . But there are some other issues surrounding all of this, I have wanted to research further and affirm with other eye witnesses and family before sharing about one extended fmaily of my parents(both sides) whose relatives and in-laws seem to have been running for President and Vice President of USA like it was the 'family' business for the last thrity years. . Politics have sometimes been in direct oppostion to each other. At least, today the ballet box has been the battle ground as in prior centuries when relaitves got in disputes whole nations went to war over family 'squabbles.' For seome reason, my parents who raised me made a concerted effort that I should not be involved. But this only put me in the eventual 'crossfire.' of all that was going on and the truth seems to be the only way for survival for myself and immediate family in all of this. Too often, individuals were never told the full truth of who they were and this has set up conflicts, etc.
World Peace does start in our individual home and extended families. Don't forget WWI involved 10 Kings, the direct descendents of Queen Victoria and Prince Albert; The ousting of one of those 10, Czar Nicholas II from Russis from power occurred and the world can't seem to get out of on going wars since then.Holocausts and horrors and less and less regard for human life and rights has occurred while in the mix has been the hope of positive changes for human rights and dignities of each of us. Spiritual warfare increases as does the one of the material world in parallel. Evil towards each of us and between us goes on along with bombs and bullets and weapons little of humnanity had imagined just a hundred years ago. False spiritual leaders of every kind of 'ism' lead the masses astray and few 'prophets' are permitted to 'blow the whistle' without retaliaiton of some of the worse behaviors and loss of life.
Many ousted royalty who had ruled with absolute and less than absolute power over the centures ended up in the USA after WWI and soon their descendents stepped right in to the world of politics and some won some of the higher positions of leadership.Many may not env know of their ancestry. Some have done well, some OKAY, but some did not learn the mistakes of their ancestors and have caused a lot of harm for all of us. Its why its so important that all citizens vote and show by the presence of the vote that the real power in the USA is still from the people who allow the leaders to hold position and can vote them right back out if they don't perform well in the job. When citizens don't vote, then the power shifts to the few with great wealth who can then hold on to the greater power;As a few ruling famlies did for thousands of years. Even in ancient Israel some Kings did what was good and right in the eyes of the Almighty and others did evil and caused the people and nation great harm.
No human is perfect and thus no candidate is either. We can only vote for whom we think will take the mantle of serving the people and not themselves in wrong endeavors.. Our voting presence keeps them aware that they do not hold supreme and absolute power and are accountable to all of us and to humanity. All our leaders profess to hold A Diety as Supreme and thus are also accuntable to the one they worship. As a Christian, I am taught that if one desires to be the greatest among us , one is to be the servant of all of us. Its an awesome and humbling honor to be voted into high office and we, the people must not let those whom we elect to forget they work for us and each of us need to vote based on whom we think will b do the best for all of usa nd preserve our basic human rights of life, liberty and happiness ( property) as some say it orignally said. We are but humans trying to judge other hmans.
The dignity of the indivuidual to have rights and each of us is cherished and loved by the Creator who made each one of us in His own image according to my book of faith The one, Yahusha, whom Christians call Christ Jesus.

So repostings of the Violations of civil rights will be done again. And we will just re sign and amend and send them in again and pray that some day human rights, the law , the constitution will be upheld which has not occrred in over a decade or more for this family...and for too many others.
