      In 2/2012, via certified mail, I filed for Part B Medicare. By 5/2012, It had not yet been processed.
I received a letter asking me if I wanted part B Medicare and I responded I did. , in writing. To date I have received no decision on my request for benefits.
I am requesting a decision with appeal rights on that request.
Part B Medicare would be my third payer for medical care.  This is another in a long list of delays in processing items. Such as requests for reconsideration that are still pending as far back as 9/2000 and 10/ 2002. Then there are waiver requests pending for years as well as due to blatant fraud committed by those in authority at the highest levels of SSA, it is not known for certain if I was ever due Medicare or a period of disability prior to age 65.. That action precluded OWCP from adopting it and seems to have bene the reaosn for the action at the Commisiosner's level according to witesses.
Several managers were ordered by some unknown High official, at the commissioner's level to not process my case. They have also not complied with a signed agreement I have with SSA, my employer. so that I can go ahead and retire which requires federal workers compensation to process the request pending with them since 1996.
Since I already have permanent medial benefits with OWCP, there should be little need for another medical coverage, but I; one of several that OIG for OPM says had the million dollars set aside money stolen from the agency and/or govt contractor, Affiliated Computer services, which is also SSA's contractor, I've been told and who has refused to post my files at SSA or at OWCP on orders of one now convicted in the Jack Abramoff Bribery scandal which is and on going investigation at SSA, US dept of Labor HHS and unknown how many other agencies.
I have been victimized several times over for doing nothing but going to work one day on 1/10/89  at HHS, Social Security.and being severely injured in a toxic cloud that has already cost lives and livelihoods for many and will medically cause many to have premature deaths on an ongoing  basis.
The signed agreement, and one has to be certain they are looking a the one with pen and ink changes and not the other one created and not signed   indicates this obstruction of laws and judges orders, etc. is not to be occurring.
If you are under orders by some party not to process my request for Part B Medicare, then I wish to have the name of that person, their job title and position and employment address. so that person can be  turned over to the professional conduct standards office at the US Justice Dept. for prosecution as this has been and on going battle for life sustaining medical care and obstruction that has cost much financial hardship as well as obstruction of medial care for my self and  especially since  1/12 for my entire family as Federal Blues is also being told by some 'unamed' part y to call the Medicare Coordination of Benefits office and have our Fed Blues deleted off the system so they can pay at the retired rate which the official posting that three Medicare judges say is accurate posts me as active duty and federal workers compensation and Federal Blues as primary  payers for entire family before Medicare This is called a RICO situation by US atty;s office as it is altering official reocrds to cause theft of Medicare, when they shoudl be sending the paid bills by medicare to the primay payers for recovery and coding them as conditional payments... But since the OWCP 'set aside monies' are missing in what OPM 's OIG said several years ago is an on going  'suspected' embezzlement ring (ACS"s call center employees can see it on their systems and weere telling medical providers we had the money ourselves but now are not allowed to mention it) they can do nothing until they are permitted to investigate. Afffiliated  Computer Servcies will not process a single oxygen claim as their manager says they would be paid and all benenfits would"have to be paid"  and refers to the convicted criminal, no longer there who said  past  Director of OPM  ordered: "Let Her die" and that has not been rescinded to date. by the current director. Its unknown if any word or letter has ever reached him for permission to investigate.
OWCP's contractor pocesses no oxygen claims, but does other I timely file but doesn not pay referring to the missing million dollars which I do not have and never did. Since no one will say who is ordering this,currently, and will not refer the name to the Office of Professional conduct themselves at the US Justice dept , an impasses exists with life and livlehood in the balance. This non  reproting of an illegal order is an illegal act in itself.
In the mean time the former partner of Medicare Trailblazer and other suborinates of South carolina Blues, created 17 claims out of each one after faxing a request to MCOB, bi weeekly to flip the Blues to secondary and paying 17 claims per claim from one ( only one pay got to provider, the other got diverted after medcicare trust fund was debited). I provided the prof of 17 numbers to your office back in 2008 when it was discovered and  no one is allowed to investigiate what the 500 named witnesses have estimated as a $4 million dollar theft of Medicare by stealing my claims and ID and over all over a trillion dollar theft on other patients they discovered the same thing had occurred in a prevalent manner. This off line computer system with no security on it is being used through out the Medicare partners to manipulate claims and was discovered by me on  2/108 and started my having to investigate to protect my self. from liability and life endagnered as diagnosisi codes altered to circumvent having to code for owcp reocovery. This became life threateniing in 2/07, when medical care got denied at a hospital and surgeies got delayed over it and other medical problems, too.. All under the orders of High level officials whom no one will name nor report themselves to the  Office of Professional Conduct to get them stopped.
In 2005, Sec Of HHS Tommy Tompkins signed a memorandum of agreement with SSA to take and process the waivers on this kind of overpayemts that 'are against equity and good consciennc.' Some of the employees I spoke with at SSA do not seem to understand that one does not provide income and resource info for this kind of waiver as obviously I can't stop this kind of theft and liability against the patient when the federal govt themselves can't do so. Non one has been permitted to process them.
OWCP needs the CA-7 completed for 1/10/89 so that the leave buy back can be effectuated and I have yet to recieve any lumpsum of annual leave and credit for sick leave so that a final pension amount can be computed.  Of course the permannet disability at OWCP must be done first and SSA is fully cooperating on? but the actions show otherwise has occurred. What I was told is some 'unnamed person from OPM' called and said "not to bother as I had gotten an OWCP setltlement and retired" Again some 'party', a major suspect in major theft and resulting deaths? is going unfettered to continue to do great harm and I need that name and position, etc. and needs to be reported for cirminnal prosecution.
 Many current employees don't seem to understand that as of 2/09 the OWCP judges ruled that I did return to work in 1990 and that benefits should be reinstated at owcp. This would then result in form 521 being filed with OPM and any csr payments refunded and then SSA Title II wife's benefits would be paid as of 12/06 which renders moot any issue pf back part B  premiums. All the proper docuemntation was filed with the CSR application to do that when OWCP finally ruled. OWCP's contractor has not cooperated at all. Evev CEO, Lynn Blodgett. told the OWCP Regional Manager in Dallas a blatant untruth.  Nothing has been processed as orderd by many judges and hearing officers and items are pending as 'New' back to 1989 on some bills and aciions and secondary and tertiary medical conditons which are rather 'text book' usual outcomes for the accepted  medical conditions of occupational asthma and  chemically induced pneumonitis after Inhalation of toxic fumes with bronchial spasms. I've wrtten many letters asking for how to proceed in a systematic way to comply with the many judges, and to date no response., except to deny everything that has occurred.  Apparently, few employees seem to know their employee rights, if  one of them became severely injured at work and had to file for Federal workers compensation.
Please help break the decade impasse in your office which is also  my employing agency where I was formally transferred to US Dept of Labor jurisdiction on 10/11/1994 after SSA' Region II finally realized that someone had played a game with papers and nearly cost me my life several times from not being reasoniably accommodated for my physical accepted injuries. of occupational asthma  and chemically induced pnemomitis which was diagnosed and submitted to them in 8/89. Even EEOC sancitons agianst HHS the dept head at the time, never seemed to get to the 'rght' officials, either.' OPM never knew I returned to work in 1990 and caused a lot more problems and to date still doesn't know about the other medical conditons and any of this and continue misinfrming Federal Blues to alter official records. . Someone has had the ability to play a real game with data and files and monies are misisng as a big ; motiive.' for skuldugery?.
Recently the entire SSA  claims file had to be reconstructed after it disappeared forever out of the McAlester hearing office in 9/09 and that, in itself is a violation of law. Issues were and still are "entitlement factors of Medicare" including the period of disability, GPO offset when one gets temporary CSR payments when owcp is involved, and other matters such as the SGA reconsideration pending since 9/2000 and An SSI claims for some periods  of back pay filed 10/2002. I udnerstand the typed , 26 page,SSA 795 did get posted to the system by ACS, but the complete file has gone partially missing again. 
Don't forget, it was a felony disappearance of fours years form  my OWCP file in Dallas that caused this all to occur back in the late 1990's and stopped the process from being concluded then. Then later, more delays as everything disappaeared that reflected I had an attorrney of record in the matter, too. So the entire file has been reconstructed multiple times and still not processed as orderd by 15 judges and two hearing officiers at US dept of Labor. Its about time that security of fies and data is taken seriously due the horrendous harm that can occur on an on going basis and allows major offenders to go unprosecuted whose actions appear as a cover up of some major felonies which have cost lives. It also indicates judges rulings are batantly and routinely ignored, too, in mulitple agencies. So geting more of them? will just be ignored more?
More recently, it was learned that SSA's computer system has been hacked into by outsiders as the posted appeals that were on the system and referred to in many administrative messages over the last decade as follow ups were done, just 'disappeared 'off the system The Head of IT for SSA and others said that cannot legally occur or be done by anyone working for SSA as an appeal requires a decision to be input. Otherwise the appeals should be on there and they obviosuly are not. But my protests about this for over a decade go unanswered and rebuffed continuously and public should be very concerned about the security of all data in the system when  what  can be done on my records,  can occur with others ad may have, too..
OIG Directors have stated to me that disappearances of data and paper files is not investigated and has not been done for  almost 25 years. That's a long pattern of behavior proving no security exists in  any federal office, at least if my files or data are there.
Felonies were committed before any Social Security , then HHS , employee entered on duty at the ADP Building at 2 Journal Square 9Th Floor Jersey City NJ owned by Hartz Mtn Industries  and deaths have occurred due to these, plus other illegal actions that have occurred. Then there is the millions misisng at HHS and at US Dept of Labor transferred to  the federal reserve that was to have arrived at OPM?? But in 2005 may have gone to Affiliated computer services to issue ATM cards that disappeared and never given to injured federal workers. One source said the ATM cards, at a million doallars a piece were cashed in and used for the bribes and still are. The documentation so far indicates that there are some at very high levels that have a grave incentive to cover up their actions and issue illgeal orders to not process a simple request for Part B medicare.
Please process the pending matters. Destruction of records is not processing. As the judge said: unitl sometihng is processed it is a NEW item and in several agencies, NEW items have been pending for many years. Some almost 25 years.
I appreciate your attention to this matter as such abuses of power should never happen to any Social Security employee ever and legally is not supposed to.  Am looking forward to a deicison on the reqwuest for part B medciare and then other items. If files need to be reconstructed, araements will need to be made with Doyle to be present as you copy them.

PRESIDENT OBAMA BORN IN TOPEKA KS: During WWII, one could not buy a new car as none were being made as war was being waged and all factories were making military vehicles.
IN 1946, my parents bought a Chevy Pick up and in 1948, my Mom got a new car to drive around town.
It was a 4 Door Fleetwood Chevrolet with Dark Blue top and grey bottom. She drove that for over 25 years taking us to schools, activities and to church and shopping. The mileage was low as trips were short. These vehicles were made sturdy and to last. And Dad always saw they were well mainiained mechanically.
Around Topeka , Ks it was a common sight to see me being taken places by my mother.  As the years went by, others got newer cars, but we never did.  In the 1960's it did have to have a new differential put in it for a few hundred doallars and it was done.
Finally in the late 1980's my parents Tracy and Florence Hardy , then living at RR Paxico Ks, sold it as a running antique. I recall for more than  the  $500 dollars they had paid for it  in 1948. Somewhere I may still have some of the papers, but other files have to be gone through for medical care and livelhood first so it may be awhile before I get to some of these matters. When my Mom died in 1996, I went through things briefly, but one did not know the significance of a car at that time.
President Obama hadn't even run for the US Senate yet.  This is the car that my Mother took Ann, the president's mother, places back in 1961.  First she took her to Topeka HS to register her after getting the guardianship papers signed over form Fred Phelps. I saw her driive and park in front of the HS and met them in the office. Mom never said much as Dad left contacts with her up to Mom. Ann had a live in job at the catholic text book store where she cold live and take care of the few customers that might come in needing a replacement textbook or other items during the school year and also stock the shelves.  and make sure the prices were on them ,etc.  For all the slander and gossip that has swirled around about her, she had a responsibility and seemed to take that responsibility seriously.
To this day, I do not know where my Cousin Daniel Wayne Pope, her Dad and mother were. All this was always so  hush hush. Obviously they were not in Liberal KS with him working in a furniture store and her as a Bank secretary.  Witnesses place him in Mercer Island when Ann was younger. And that is where His maternal grandmother was in 1/71 when he returned from Indonesia. over concern for his education, I was told.
IN 8/1961, Mom left our home at 1748 Randolph St in Topeka Ks to go pick up Ann and take her to Forbes Air force base hospital where she had free medical care to have the President.  As I've written her before, she did not get home until later the next day. She went to bed to get some rest. She never said anything about the delivery or what had occurred.
The next day, I had to go to a music lesson and went out to get in the car. Mom opened the real drivers seat door and realized she had not removed the blanket that was in the back.  There was a big stain and part of it had soaked into the stuffing in the back seat. There were rayon cloth type seat covers. These were in good shape up unitl the time they sold the car as rarely did any one ever ride in the back seat. I usually rode up front. I recall Mom did smell of it and it was not urine, it was amniotic fluid. Ann's water had broke in the back seat on the  way to the hospital.  She took the blanket inside and wiped the seat some. There was always a slight stain and the fluid had gone into the stuffing.
I'm not an expert in DNA, but it may still be there and someone probably still owns that car. You may have the place where the birth of President Obama began. Obviously there was no time to fly to Hawaii nor to Kenya, a false bio concocted by the fall of 1963 by others than the President. Does he know, yet? I have no way of knowing. I do know that as of 1/71, his maternal grandmother stopped me from saying anything to him about Topeka Ks and seemed very frightened and shook that I was present at my cousin Rolnad Hackett's house when they arrived.  I was never any part of anything or any decisions in the matter surrounding the birth. yet, my life has been changed over it. When one is an eye witness to events and then terrible things are created about it that are not true, silence just can't continue.
My knowledge and impression of Ann was  a young woman put in a situation that she did the best she could and made a decision to live and care for her child.  What transpired that led her to go out to Mercer Island by the time HS started in the Fall ofter giving birth, I do not know, I do know, that the gossip frenzy in the town had died down after the deaths, but assuredly would have been resurrected and obviously safety of both Ann and her child, President Obama ,would have been of concern, once customers started arriving in a short time to buy Fall textbooks.
Most people were a decent sort, but I knew myself just how vicious some of those I could go to school with could be and some of the people in the city. No one stood up to then.All the bad things that happened in the struggle for integration seemed to be lost in some kind of  'collective amnesia' to this very day. And from time to time I have heard of things occurring in that city, that indicate some of the prime suspects may still be exerting power snd authority in a less than ethical way. Its become prevalent in too much of society and currently, am experiencing some of those same abuses of power in my own personal life and struggle for medial care and benefits that have been established and should not be happening.
