ANALYSIS OF BILLS
11. no mention of my two primary payers that show on the Medicare computer which receives an official secured official posting bi weekly from US dept of Labor by contract between Emblem Health's subsidiaryand them.
 Nor does this indicate that it also shows active duty status. No one is permitted to violate that posting. And three Medicare Judges have ruled it is accurate.
2.I have refiled for Part B Medicare and Social society refuses to process it since 2/2011 due to computers system that a month ago was determined have been hacked into by outside parties illegally for over a decade on me. IT Security is attempting to get OIG for SSA to do something about this. Apparently, there is a Nationwide policy in effect for at least 6 years not to bother with security of paper or data of claimants records. which also includes personnel files of federal workers.
3. If this is the payment form Medicare Part B, its news to me that they now have fixed security problems and expect to be receiving a medicare card soon.
4. Both times I came the forms I completed and signed listed Medicare part B as third payers.
5. The specialist that ordered these blood tests is treating my for a condition related to my original accepted conditions at Federal Workers compensation.. but the secondary conditions which have been accepted by others with similar original injuries  has yet to be reviewed since 2004 when I was diagnosed and treatment started although the medical records clearly show a progressions of  disease starting with the date of injury of 1/10/89.
6. Medicare's computer owned by the private contractor rarely reflects the correct date of injury as it shows the date of entitlement to part A Medicare which , for most would be 30 months after the date of injury which causes a major problem in the computer automatically setting claims paid up for recovery from the liable party which is federal workers compensation at some point.
7.There is a delay in processing all claims due to the million dallars that shows on the system at Affiliated Computer Services which process data and files for both Social Security and for the US Department of Labor as well as for most other federal and even state agencies and those in other Nations through various trade treaties of the last decade. This million went missing and is part of the Jack Abramoff bribery scandal that is on onging in HHS and US Dept of Labor ands SSA and other agencies. (few seem to understand this was/is govt wide.) And indications are that the bribery money came form the set aside medical care for many Federal workers including mine. It disappeared before getting put in a fund at Office Of Personnel management and when I first became aware of this as ACS was telling my oxygen supplier and medical providers I had the million dollars, I wrote letters (still unanswered) to say I did not have it and wanting copies, etc and ended up speaking to an Ingvestigator for OIG for OPM who blurted out: That they had suspected an embezzlement ring for some time but could not get permission from the Directors of OPM to investigate. The prior director Kay Cole James left about the time of discovery in 2005? and so did Horace M Cooper  over at US Dept of Labor who has since pled guilty for not reporting all the wining and dining and sports tickets from Jack Abramoff but so far no one has found the money and appears the Current Director John Barry was never told by the OIG for OPM? and my letters to him have never gotten to him . The prior director Linda Springer did start making phone calls about the situation on my records , etc and then abruptly resigned in 2008 to work for Earnst and Young, one of the two govt contractors in charge of spending the $700 billion d TARP monies that Congress still has not been able to find out for sure what it was spent on.

You show one MEDCIARE NUMBER WHICH BY OFFICIAL PROCEDURES AT SSA WHERE I;VE BEEN LEGALLY EMPLOYED SINCE 1968(paper personnel file) AND CAN'T RETIRE UNTIL OWCP GETS ME ON PERMAMENT DISABILITY WHICH I FILED FOR IN 1996 AND ALL MEDICAL RECORDS AND SUPPORTING DOCUMENTATION IN VOLUMES  WERE IN FILE. but Horace M Cooper said because I was old it would not be posted and said Director of OPM (Kay Cole James) said :Let her die!  The manager of TX  Blue Cross Blue Shield said the prior director Constance Horner had first given the order and it was also given? to you back in 5/2000.
9. Part of the cause of the massive confusion that has occurred on my computer records inside three federal health plans is the outcome that is on going that started beforee 5/2000 and has contributed to it. All started with a disappearance of four years of my paper file at OWCP and the theft of files was so grave  on so many cliamants that they ended up contracting out to ACS, as security just couldn't be provided was the respsonse. ( I just repeat what I'm told by those in authorirty.) Being a victim of this has only resulted in worse problems occurring.

Where  and why do you show Insurance as Medicare part B?
Did this come from Medciare directly ? Is\f so , who and where?
Did it come from your clearing house which billing services and yours would be contracted with?
If so which one and who or what was accessed to get it?
Since your incorrect payment is part of the on gong bribery scandal with millions missing using my records and info alone, its imperative to know who and where this info came from as it was not provided by me.
11. The other problem is a constant changing of diagnosis codes and even tests ordered to circumvent the liability case on many patients. The 90% reduction in fee would indicate something like this has again occurred.
12. Was Federal blues billed  first as that was the legal info given when I came in for the tests?
If not, who was contacted that indicated your billing [personnel should not do so? need names and company names?
13. although I went to a local facility for the  blood tests, the return address is Atlanta Ga.
Is this a subsidiary or interlocking company of Affiliated Computer services?
14 Apparently the information provided was ether not given to your billing service or they chose to ignore it along with the sentence that I added over my signature that I would not be liable for bills not sent to the legal parities liable and appealed if not properly paid.) This seems to  appears that your service is doing a disservice to your facility and resulted in non proper payments. What happened to those forms and if they had them why were they ignored?
15. What health plans were billed and what was the result form each/ Each need to be for protective filing dates.
16. Did either Medicare or Blues instruct you as to how to code the claim for a conditional payemnt so recovery could be instituted from OWCP? if not why not? Need names and company names of contacts.

Your  medical staff was very professional in drawing the blood and getting the test results to my doctors, which are shared with pulmnologist, cardiologist kidney and family doctors to reduce multiple tests needing to be run for treatment by each.
Your  attending to this is imperative.
17. There were/are 17 Medicare numbers created by trailblazers and each claim paid 17 times back to 1/06-started in 8/08. Only three of the claim numbers came from SSA. Its unfortunate that OIG for medicare was never permitted to investigate. The investigators assigned form the HHS and for the US Justice Dept stated that Congress would not permit any investigations. I read that 1000 in North TX and southern OK are losing their jobs for non renewal of contract but, also read that the top employees have now gotten jobs with the new Contractor Novartis  which bought out High Mark that had received the contract . During the 2 years, 1-800 Medicare 's employees and I were investigating this and  500 fraud reports diligently completed by that contractor they indicate that The contractor for Florida  was also creating multiples out of one on the illegal offline system being used throughout the country by the claims processing contractors of Medicare. But only SC Blue and subsidiaries and Florida were ever mentioned as  being known to be doing this on many other patients.  The internal estimated theft was a trillion dollars. $4 million by stealng my claims and Id and turning them into multiples while they also ere illegally asking Medicareciar coordination of benefits to flip Medicare to primary payer, then recoding claims so it appeared to not by OWCP and then often Blues never got notified to pay the co pays which I recently did and years of back  co pays got paid to many providers and supplires. who never resent. None were coded for recovery from OWCP except paid oxygen claims from medicare Cigna Govt Services; and that's illegal.  Since 2009, 1-800- Medicare as been instructed by their new compnay General Dynamics to not see fraud any more. nor report it. on any one.
The Medicare trust was debited but where is the money that went to the federal reserve to be sent out to each medical provder who only received one pay? Audit the federal reserve is going to provide a data trail leading to many being arrested? if Congress wil ljust pass the law. It will show who diverted monies and where they went.

               An eye witness sharing is being done first in these postings. Its how one learns a family history.
Some of the parties are alive, but many I have never met except through being told about them from other family members I did know. Fortunately, little was ever shared that would be 'bad'. Most shared the things they were proud of that their children or grandchildren were accomplishing.
The family on my Dad's side are the family of the president that he may never have known or known about.
Many of them, that did know somehting may never have been told of his origins in Topeka KS. It was deferred to the immediate family to share what they wanted to share and it was all the concocted Hawaii story. Reasons could be well understood as explained here previously.
In the modern era, one does need to know where their official records are, and some of the basics. At least after they become adults. As I have shared before, if one is raised by those who lave and care for them, then one needs to feel Blessed and have forgiveness for them not sharing. Many came from an era where ones papers were not needed very much.
Aunt Pearl was very proud of her children and shared much about them with me directly , but mostly with my mother as they wrote  and called each other. Aunt Pearl called me after Mom died but I didin't follow up and few understood the life and death struggle we were gong through and still are. I should have said more. At the time, I was in a battlle trying to stop 9/11 as what I knew was being rebuffed on all sides and retatations only getting worse over it. ( Posted this on Daily recap 9/11/2012) Now,  its well known, many like me were going through the same thing and none of those in charge seemed to be so. I should not forgotten to continue the ties to family that my\mother had done so well with. On Doyle's side, his older sister, who had been the one in his family of sharing the famiiy news, also died of colon cancer and it was a double whammy. Both of us are the youngest in our immedaite famalies and often the younger don't have the expectations to 'carry on.' When the horrible things were being said about the president's mother, my second cousin, and all the wild speculations, I could not be still and began posting on 7/26/12 as I realized many did not know what I knew that may still be alive. No one is perfect, but when in the political arena when rumor and gossip gets so ugly, one must do something. This is not a political endorsement and each should vote and ones vote shouldn't be based on such.
Tracy Vallier PHD Geologist, has explored and mapped many areas of the world. At one point he was teaching at the U of Oregon in Portland. My Mom wrote and said he had moved his famly to northern California and taken a job with he govt as he was trying to warn folks in his area that there was going to be a volcanic eruption and he wanted to get his family to safety None were lisitening to him!. I did not know which Mt or if he said which, but soon after they moved. Mt St Helena, blew. He had had a grant and mapped much of the Northwest. Aunt Pearl said that at one time he had seem some indication of Big Foot , but no direct sighting. But did suuspect something was out in those woods.  Some of his books are available at the main book sites for purchase. I had jokingly said to my parents at the time in the early 1980's. Let me know when he leaves California as it may be time for everyone to leave.
He later was sent to Armenia in Russia to do some kind of research there and soon after, there was a major earthquake in that area. Later, he was in charge of the main research ship that was doing research around the world and its mentioned on line , too. I just knew it was really important work to try and determine unstable parts of this world were both on land and under the seas. I have never met him personally, yet seem to know much about him though his parents and mine as news was shared. I have pictures, too of his family.  At one time, Aunt Pearl visited and wanted to ask my dad about a son of his( pretty sure it was) and a family physical disease and they went and talked some in private and this time I was not in eavesdropping vicinity. Although I often overheard many things, it was';t trying to do so .Iit just happened I would walk in and hear things, or some would start talking not realizing I was within ear shot.
