There's a cover sheet for each category. Since the total documents are voluminous and can't be sent all at once due to physical disability limitations, and the need to meet filing deadlines; the cover sheet will be placed on top of whatever category is under. This has been growing exponentially since the first filing and no investigations has only made it worse including obstruction of medical care. Doyle @ Linda Joy Adams+M.

UPDATE; No response allowed to be made to my family and I on the civil rights compalinsts filed with the Dallass Civil rights office since 1/08. We refile and amend ever so often. There are no procedures in place for us to go beyond the local office until they respond. No Intake numbers on Civil Rights ever done and none on HIPPA in recent years. Only the certified mail receipts listing the civil rights and HIPPA complaints is the only proof they were ever filed in the last few years on new incidents that keep occurring and was told ACS-Xerox is getting them. Social Security now under him? And it continues...from agency to agency including many state agencies where this occurs.. first to scan in and just another govt office where our files are withheld from view so cover-ups of gross crimes against humanity and denial of constituional rights to stop it in the USA are covered up as they are representative of what is getting more and mroe common place in many ways.
ACS-Xerox's CEO LYnn Blodgett who has given out false info to Christina Starke Mgr at Federal Workers comp and she has to believe him as she can't have the files., there.
My employer, SSA was lied to saying I had receveda settlemtn and nothing more to do when they still owe me buy back leave and other things to be done they agreed to do in a signed agreement with them HIPPAA does little good, as govt officials and govt contractors are not covered under that and where the offenders are for the most part. The impasses and chaos created has caused terrible obstruction of life sustaining madical care and reports are that others have died from such recently. These complaints would have exposed all and corrective action taken so policies and procedures would be required to havebeen made and lives and livelihoods helped for so many affected, not just us. FDA is named in many ways, for example, not just health plans., etc.
For the most part we consider our medical providers victims of the same corrupted system which seems little hope of getting corrected until HR 1063 is implemented. First is educating all those involved what a conditional payment is under the orginal 1981 law. with enforcment susuepnded since 1994. -legal enforcement of payment when the coverage already exists by the primaries; now with fines and penalties added on. Medicare can pay if coded for recovery, etc. Patient and doctor, etc. have no liability as the govt's is to enforce the laws, not us.
In 1999, Medicare Coordination of Benefits was contracted to Group Health and soon had illegal 'deals' with the offenders to faciliate the bad acts in ever increasing means and vast amounts of money missing.
\US attorney's calls it RICCO, ORGANIZED CRIME RUNNING MEDICARE.
The new law puts $1000 a day penalties on them and why OIG used to be part of this. ITS THEFT!
We refile and add and amed new dates and events and Debbie C in Dallas OCH-HHS has e mails alrady. HHS was the employer at time of injury as SSA was part of it. Is this retaliation for pending matters and EEOC SANCTIONS obtained against them before. I allege, YES!

Update: although I send nearly daily e mails to the Office of Civil Rights for HHS in Dallas no one ever responds and years of civil rights complaints do not get docket numbers or answered as its been said Lynn Blodgett the dictator of the USA , the hired hand in the USA of the international cabal of companies and illegal secret deak partnerships will not permit it. We refile to keep all legally alive. Must be that stack in his personal office of unprocessed oxygen claims since he took total control of FECA in 2002, along with those not process from Celerian Group the Medicare intermedicary AND Illinois bLUES, ETC OF WHICH HE MANAGES ALL THROUGH THIS MAZE OF SHELL COMPANIES . AND DEALS. , Plus all the SSA-632s waivers of Medicare. etc.unprocessed as its waiving what I got no benefit from.
There is no process to go forward without that initial responses. HIPPAA does not help as government contractors not under that. NO RIGHTS TO MEDICAL CARE BEING LEGALLY PAID, ETC UNDER REAL DEATH PANELS CREATED BY CONGRESS IN THE COUP OF 2002 when internal audits of government contractors as even health insurers for every one are now, criminal investigations with trillions stolen insiders say, and SEC not able unravel the maze of maybe 200 companies are more now as CONGRESS DEFUNDED THE RIGHT OF THE REAL GOVERNMENT TO DO ITS JOB IN THE E GOVERNING LAWS OF 2002. Then self imposed an unconstitutional gag order so they could not restore the real government to power or help nearly any consitutent as the issues usually are with the govt contactor, as they forbid any member of congress from mentioning, contactint or having one testify to know what is really going on so that laws they passed are based on facts and real data and not the reports they file that cover up their on going wrong doings. We have elected too many who have chosen to not wish to know and stand up to real evil. Those that wish to do not have the support of the voters , who most do not even vote to help them in the fight for freedom and justice and restoration of the US Constitutional rights of each to have a government of, by and for the people that works for US and has not abdicated to the control of this beast system that has nearly taken the whole world people and nations; governments now. Waiting passage of the Trans Pacific Asia partnership to ship all the data on people and nations to an offshore company that will never let any real government know what is done and one man can control the world from one server soon. A power no one human should ever be given. The founders of the USA knew better and so fo most naitons now and have checks and balances on power to curtail when one in authority abuses the power and can be peacefully stopped. With 38 federal judges from multiple agencies defied by Lynn Blodgett over all, and no full US Supreme Court seated at the top, the judiciary branch is not even in power few can ever get a judge at any level obeyed now. The Congress has gagged itself to not be able to discuss and vote on restoration of the real government and over sight so they cannot help. And the President cannot act to help as congress holds the purse strings for any executive order to be carried out. or any order to enforce the existing laws, cannot be done as it cost money that the Speakers of the House from 2002 to the present have not allowed to even earmark a portion of existing budgets to use for these basic oversights of government contractors.(DOD kept theirs and recently it got in for the VA somehow due to Sen Sanders, his aides said) The taxpayers money is only spent as okayed by the House. and I name the head as responsible to see its done. NO RIGHTS EXIST UNTIL WE HAVE A GOVERNMENT IN PLACE AGAIN THAT WILL ALLOW THE PROCESS TO PROCEED so justice can be had for each and every one of us. Until then we are under an overthrown government and few even are allowed to know as this cabal illegally says they are the real government and are not and are not allowed to say they are.
